6 Extra Help Prescription Costs Seniors Can Lower

Medicare’s Extra Help program can reduce Part D premiums, deductibles, copays, and other prescription drug costs for qualifying seniors, helping stretch retirement budgets further. Prathankarnpap/Shutterstock

Every year, thousands of Medicare beneficiaries pay more for prescriptions than necessary simply because they don’t realize they qualify for Medicare’s Extra Help program. Others assume they earn too much or own too many assets to qualify without ever checking the current limits. Because income and resource thresholds are updated periodically, someone who didn’t qualify a few years ago may qualify today.

If you’re paying more than expected at the pharmacy, Extra Help may save you hundreds or even thousands of dollars each year. Before assuming high prescription bills are unavoidable, here are six prescription costs seniors can lower now.

1. Monthly Medicare Part D Premiums

One of the biggest expenses Extra Help prescription costs can reduce is your monthly Medicare Part D premium. Eligible beneficiaries may pay little or no premium, depending on the drug plan they choose and the benchmark premium in their state. Lowering or eliminating this recurring monthly bill can free up room in a fixed retirement budget for groceries, utilities, or other essential expenses. Even if you already have Part D coverage, qualifying for Extra Help can substantially reduce what you pay going forward.

2. Annual Part D Deductibles

Many Medicare drug plans require beneficiaries to meet an annual deductible before coverage begins. For seniors enrolled in Extra Help, that deductible is generally reduced to $0, allowing prescription coverage to begin immediately instead of waiting until the deductible has been satisfied. This can make a significant difference early in the year, particularly for someone taking expensive maintenance medications. Rather than delaying prescriptions because of upfront costs, beneficiaries can begin receiving lower-cost medications from their first eligible fill.

3. Copayments for Generic and Brand-Name Medications

Extra Help also limits how much qualifying beneficiaries pay for covered prescriptions at the pharmacy counter. In 2026, Medicare states that most beneficiaries receiving Extra Help will pay no more than $5.10 for covered generic drugs and $12.65 for covered brand-name drugs, with some individuals qualifying for even lower copayments depending on their Medicaid status. These predictable copays make budgeting much easier than facing widely varying retail drug prices. Someone who previously paid $60 or $100 for a monthly prescription may see dramatic savings after qualifying.

4. Late Enrollment Penalties

Many Medicare beneficiaries worry about enrolling late in a Part D prescription drug plan because Medicare generally imposes a lifelong late enrollment penalty. However, individuals who qualify for Extra Help don’t have to pay the Part D late enrollment penalty while receiving the benefit. That can save money every month for years, especially for seniors who delayed enrolling because they believed they couldn’t afford prescription coverage. The penalty isn’t simply a one-time fee; it normally continues as part of the monthly premium for as long as Part D coverage is maintained.

5. Catastrophic Prescription Costs

For seniors with very high prescription expenses, Extra Help provides even greater protection. Once qualifying beneficiaries reach Medicare’s annual drug spending threshold, they pay $0 for covered medications for the remainder of the year. This protection is especially valuable for people managing conditions such as cancer, rheumatoid arthritis, multiple sclerosis, or other illnesses requiring high-cost specialty medications. Without this safeguard, prescription expenses could otherwise become financially overwhelming despite having Medicare drug coverage.

6. Costs Associated With Switching or Finding Better Coverage

Many seniors mistakenly believe they must remain in the same Medicare drug plan all year, even if another plan offers better prescription coverage. Beneficiaries receiving Extra Help qualify for Special Enrollment Periods that generally allow them to change Medicare Part D plans outside the standard annual enrollment window.

That flexibility can lower future Extra Help prescription costs by allowing beneficiaries to move into plans that better match their current medications and preferred pharmacies. If your prescriptions change during the year, being able to switch plans may produce additional savings that aren’t available to other beneficiaries.

Even with Extra Help, not every Medicare Part D plan covers the same medications or pharmacies. Comparing plans periodically can reduce your out-of-pocket costs even further if your prescriptions or preferred pharmacy change during the year.

You May Qualify If You:

Have Medicare Part D.
Have limited income and resources.
Receive Medicaid, SSI, or help through a Medicare Savings Program (automatic qualification).
Haven’t checked your eligibility recently.
Struggle to afford prescription medications.

Since changes expanded the program, everyone who qualifies now receives the full Extra Help benefit rather than the previous partial subsidy available in earlier years. That means qualifying beneficiaries can receive broader assistance with premiums, deductibles, and prescription costs than they may remember from the past.

It should also be noted that some people will qualify automatically, while others will have to apply through Social Security.

Extra Help Doesn’t:

Cover medications not included in your Part D plan.
Replace Medicare Part B.
Cover over-the-counter medications.
Eliminate every prescription-related expense in every situation.

Why Checking Your Eligibility Can Pay Off

Prescription costs don’t have to become an unavoidable part of retirement. Medicare’s Extra Help program exists specifically to reduce the financial burden of Part D coverage for people with limited income and resources, yet many eligible beneficiaries never apply. Reviewing your eligibility (even if you’ve been denied in the past) could be one of the simplest ways to lower your healthcare costs and make your retirement budget go further.
