Medicare to Increase Payments for Inpatient Rehabilitation Facilities in FY 2027

The Centers for Medicare & Medicaid Services (CMS) has published a final rule updating payment rates for inpatient rehabilitation facilities (IRFs) under Medicare for federal fiscal year 2027, effective October 1, 2026. The rule was published in the Federal Register on August 3, 2026.

CMS Finalizes FY 2027 Payment Update

Inpatient rehabilitation facilities provide intensive, hospital-level rehabilitative care to Medicare patients recovering from serious conditions such as strokes, hip fractures, and major joint replacements. Payments to these facilities are made through a prospective payment system, meaning Medicare sets rates in advance based on patient characteristics and other factors.

What’s Changing for Rehabilitation Facilities

According to the official CMS announcement, the final rule includes updates to the market basket payment rate and a productivity adjustment for fiscal year 2027. The rule also updates the case-mix group relative weights and average length-of-stay values, which are used to calculate how much Medicare pays for individual patient stays based on the complexity and resource needs of each case.

The rule addresses several additional technical adjustments. These include updates to the labor-related share of payments, changes to the wage index used to account for geographic differences in labor costs, and a methodology for budget-neutral wage adjustment. Notably, the rule finalizes the third and final year of a phase-out of a rural adjustment that had previously provided additional payments to IRFs located in rural areas.

CMS also updated the payments for high-cost outlier cases — situations where a patient’s care costs significantly exceed the standard payment threshold — as part of the fiscal year 2027 changes.

In addition to payment rate updates, the final rule includes changes to the IRF Quality Reporting Program. This program requires inpatient rehabilitation facilities to report data on the quality of care they provide; facilities that do not meet reporting requirements may face reduced Medicare payment updates.

The rule spans 51 pages in the Federal Register and covers regulations under Title 42 of the Code of Federal Regulations.

These changes are relevant to Medicare beneficiaries who receive care in inpatient rehabilitation facilities, as well as to the facilities themselves. Payment policy changes can influence facility operations and the availability of services in different geographic areas.
